# How to Use Historical Insights in Care Insights

## See what was actioned on a patient's insights, why, and when, without leaving the active workflow.

**Overview**

Historical Insights brings previously actioned insights into view directly within Care Insights, so users can see what was acted on, why, and when, without disrupting the active workflow. Every record is attributed and read-only.

Historical Insights appears in its own tab beside Current Insights and is available to any user with read-only or read/write Care Insights access.

**Why use Historical Insights?**

Without visibility into insights that were previously actioned in Care Insights, users can face blind spots at the point of care that lead to missed follow-ups, duplicate outreach, or incomplete documentation. Historical Insights makes that activity visible, so users can see how a patient's profile has changed over time and confirm whether prior actions led to follow-up.

**How it works**

- **Open a patient record.** Care Insights loads as usual when the chart opens, with a **Historical Insights** tab beside **Current Insights**.
- **History loads independently.** Prior activity is fetched in the background, so current workflow performance is unaffected.
- **Review insight history.** Records are read-only. An actioned insight appears in Historical Insights right away and stays in Current Insights until the end of the day.

**Key capabilities**

- **Read-only by design.** Full context with zero workflow disruption and no accidental re-actioning.
- **All insight categories.** Historical Insights covers every category, including Diagnosis Gaps, Quality, Risk, Utilization, and more.
- **Multi-source merged view.** Related insights display as a single card, consistent with the active insight experience.
- **Group and sort.** Group by Category/Measure or Code so one condition or measure reads as a single set. Sort by last updated or code.
- **Filter by year.** A multi-select year filter narrows a long history to the period in question.
- **Full context preserved.** Each record shows the insight name and category, data and source, suggested and selected codes, the action taken with its reason, the user, the rendering provider, and dates.

- **Care team members** track whether prior actions led to follow-up and spot recurring or reopened insights.

**Frequently asked questions**

- **Can I take action on an insight from Historical Insights?**  
  No. Historical records are read-only by design, so prior insights can't be accidentally re-actioned.
- **When does an actioned insight show up in Historical Insights?**  
  Right away. The insight also stays in Current Insights until the end of the day.
- **Will loading history slow down Care Insights?**  
  No. Prior activity is fetched in the background, independently of current insights, so the active workflow isn't affected.
- **Which insight types appear in Historical Insights?**  
  All Care Insights categories, including Diagnosis Gaps, Quality, Risk, and Utilization.
- **Who can see Historical Insights?**  
  Any user with read-only or read/write Care Insights access. It's hidden for users without Care Insights access. Contact your organization's Vim console administrator to update permissions in [console](https://console.getvim.com/organization-admin/applications/gaps/app-store).
