Use it between visits to find who needs care, decide who comes first, and build a list for outreach. - **Your whole patient list.** Every patient with open insights, not only the patients on today's schedule. - **Access in Care Insights.** The same Vim app you already use in your EHR. No new portal and no new login. - **Open it any time.** Population Health is available whether or not a patient's chart is open. ### How it works: What you can do - **See the totals.** Cards count patients with ADT events, quality insights and diagnosis insights, and update as you filter. - **Filter and search.** Narrow by diagnosis gaps, quality measure, ADT event or other insight categories. Search by patient, condition or provider. - **Start at the top.** Patients recently admitted, discharged or transferred carry a red ADT label and come first, then patients with the most open insights. - **Fill in contact details.** On a filtered list, click “Sync data” to pull provider, phone and last / next visit from your EHR for the patients on the page. - **Review a patient.** Expand a row to see every open insight for that patient. - **Export to CSV.** Download the list exactly as filtered, for outreach or your own tracking. **Illustrative view.** **![population-health-screen-wide](https://knowledgebase.getvim.com/hs-fs/hubfs/population-health-screen-wide.png?width=670&height=255&name=population-health-screen-wide.png)** ### Tips - **Filter before you sync.** “Sync data” works on a filtered list, one page at a time. Go to the next page and sync again for more patients. - **Open the chart to act.** Agree, dismiss and comment work for the patient whose chart is open in your EHR. Other rows are view-only. - **Where it opens.** With no chart open, Care Insights opens on Population Health.
